Some nurses nearby are tossing a 申 wheel back and forth as part of a game... and accidentally toss it right into Shrek's 口 mouth. Shrek rolls around in pain, 呻 groaning, as the 申 wheel broke a couple of teeth in his 口 mouth. And the 申 wheel is still stuck in his 口 mouth. The nurses try to yank it out, and Shrek 呻 groans in pain.
