Apparently this patient, who walks with Yoshi around the infirmary lawn, has a horrible 姻 marriage with a 大 big 口 mouthed 女 woman. Yoshi feels bad for this 姻 marriage, but knows there are many like it. Luckily dinosaurs like him don't even have 姻 marriage.
